Please ensure that you contact your pharmacy benefit manager (PBM), as they handle prescription prior authorizations rather than your insurance network. Your PBM’s contact information should be listed on your insurance card, although it may sometimes appear on a separate card. Common PBMs include Express Scripts, CVS Caremark, and Optum Rx.
If you have contacted your PBM and they do not have a record of your prior authorization, despite receiving communication from us that it was submitted, please call us at 1-800-221-5140.
